Part 1 of the report of the Cullen Inquiry into the collision between a Thames Train turbo commuter train and a First Great Western High Speed Train at Ladbroke Grove; in which 301 people died.
The primary cause of the crash was the failure of Driver Hodder of the Thames Turbo to stop at the infamous signal SN109, which was showing red. The death and injury toll was exacerbated by the fire which broke out when the HST diesel fuel ignited.
Some of the recommendations in the report are sensible; particularly relating to the failures surrounding the inadequate sighting of SN109, particularly after the line was electrified in 1996. Others are less so.
