"As the 4.15 p.m. up passenger train from Windsor to Paddington, consisting of engine (No. 238) and tender, and 11 passenger vehicles, all fitted with the automatic vacuum brake, was running between Acton West signal cabin and Acton station at about 4.40 p.m., the right-hand inside connecting-rod of the engine broke, and the piece remaining attached to the big-end pierced the fire-box, causing an escape of steam and water through the fire-box onto the footplate. Both the driver and the fireman who jumped off or were thrown off the engine, were so badly scalded before they could escape from the footplate that they died in hospital on the following day."
